Terms of Service

Please refer to the following policies for all services and sales from the online channels, and also land-based. 

Consultation

15 minute complimentary online consultation is offered 1x per person 

Additional consulting online or in-clinic is available subject to availability. for  $75  - for up to 30 minutes. A 30-day Quote / Approval To Book/ Treatment Plan may be issued by the Clinician following consultation and screening if procedure is deemed appropriate for the client

Medical Screening. 

Medical screening/disclosure form is necessary for most procedures to ensure suitability for treatment. An updated consultation may be required from time to time for ongoing clients. Changes to medical conditions should be reported.

Booking Process and Reservation Fee

Once a date is agreed upon,  New Clients will receive an online link to complete  reservation and establish their account, and are responsible to make note of date/time. Payments and  online agreement forms must be submitted on date provided in order to confirm the reservation.  Link will expire the same day.

Credit card on file required establish and secure accounts and confirm appointments. Further ordered products will be charged to your account.

Those who have not provided a valid credit card are required to pay via etransfer or cash in full at time of every booking

Future bookings can be made directly by Administration at your request. Consent forms are required submitted on same day of request to confirm appointment. 

Reservation/Administrative  Fees

These are separate from procedural cost and are non-refundable/ nor transferrable to additional dates/procedures as per  Treatment Plan Contract. These Fees  offset administrative  costs/clinical time lost by the clinic in the event of cancellation or no shows. These apply to ALL regular bookings, accounts are secured by credit card/prepayment.

Account Balances

Account balance must be fully paid 1 business day in advance of appointment and is non-redundable/ nor transferrable. If the card on file is declined, the appointment will be cancelled. For treatments under $300, payment in full is due at time of booking/non-refundable/nor transferrable.

Cancellations and No-Shows

Please - DO NOT CANCEL UNLESS ABSOLUTELY NECESSARY! Plan accordingly and carefully.   We commit consulting and clinical  time, supplies and administrative work for each appointment, which is reserved in-trust while we turn others away from the costly,  reserved time slots. Clients who cancel due to social engagements incurr fees as noted. 

Clients are provided with a link in their confirmation emails and texts - by which they may cancel. 

Rescheduling: Clients who cancel their original Regular Procedure appointment, at least 2 business days prior to their session, may  request to reschedule.  Reschedules are not guaranteed to be available. We may, at our discretion, offer a 2 week grace period of original reservation date to reschedule - otherwise a new appointment is required - New Reservation fees apply for each new reservation. Rescheduling may  be  offered  one time -  for the same procedure only.

Illness Credit: Prepaid  Regular single procedures only -  if cancelled with adequate notice due to a medical condition - and a medical note provided, may be issued a non-transferrable credit note  for the same procedure valid for 1yr at clinic discretion. Reservation fees apply per, and are not issued credit. *Update:  If you have any symptoms of an illness, or have a communicable illness, do NOT  come to the clinic, contact us.

Healing Assessments: may/may not  be rescheduled at clinic discretion. Limited time specials or promo sessions are not rescheduled/nor refundable.

Late Cancellations Policy:  

Within 2 business days: 50% balance of ordered treatment is payable,  and billed to credit card on file, including  fees outstanding as per Treatment Plan Contract. $100 Reservation/Cancellation Fees applies for complimentary procedures. Clients are required  prepay the full balance to schedule future appointments.

Within 1 business day: 100% balance of ordered treatment  is payable, and billed to credit card on file, including   fees outstanding as per Treatment Plan Contract.  $100 Reservation/Cancellation Fees applies for complimentary procedures.

2x late cancel/no show: or repeated reschedules may required to prepay all requested services or may be referred out. Clients with history of  cancellations are required  prepay  full balance due to schedule future appointments. No exceptions. These fees cannot be transferred to a new appointment, new appointments will be charged regular prices, also.

Late cancellation fees will be applied to accounts automatically.

Late Cancellations/no shows on complimentary or bonus sessions renders us unable to  offer additional future complementary services/products. 

IN THE EVENT OF AN UNFORSEEN CLINIC CLOSURE: due to official inclement weather travel advisories/equipment maintenance or  scheduling conflict we contact clients to provide alternate dates without penalty. 

Payment

Payment  agreements are outlined in Treatment Plan Contract. Payment in-full required prior to commencement of  any procedure. 

There are no refunds due to the nature of the procedures. Unpaid balances  sent to collection. 

Products are non-refundable, and special orders must be paid in full at time of order/billed to card on file. This includes special items added on to treatment by client request.

Regular Procedures

A regular procedure is one full-length, full-price, standard procedure. This does not include discounted, mini, custom, corrective, colorboost, touchup, multi package or otherwise. Ask for clarification.

Pricing

No prices/fees are guaranteed, and  subject to change without notice. We do not routinely offer nor are we obligated to provide any free products, free reservations, nor supplies. Special offers are limited time and availability. Complimentary sessions may be offered or cancelled at clinic discretion with no cash/refundable/transferrable value and no future obligation.

Treatment Plan

Treatment plans  and records are created for every client and consist of:

  • Consultation/Medical Screening Disclosure and Considerations

  • 30-day Quote and Approval to Book 

  • Reservation Fees/Application Fees

  • Preparation Instructions 

  • Pre/Peri/Post Clinical Photos

  • Informed Consent Agreement

  • Treatment Plan Contract 
  • Presentation  and Treatment Observances

  • Aftercare Instructions and Healing Observances/Side Effects

  • Maintenance Agreement/Instructions and Timelines

  • 1 Year Current Client VIP Status from last treatment  if applicable

Quote refers to the Treatment Plan: client requests, clinic recommendations, treatment timeline, pricing, fees, and maintenance. Treatment plans are contracted terms, and are not re-negotiated after agreement. Should a client wish to discontinue treatment prior to completion, or is non-compliant or does not abide by terms of service or  contract terms, the contract will be void with no  refund nor any future service obligations. 

Application 

Informed Consent Agreement is executed prior to application. This contract must be completed  with each subsequent application. 

A regular procedure is ONE application at full-price, full-length procedure. See REGULAR  PROCEDURES above for more details.

Corrective procedures custom and targeted applications are by quote and subject to change.

All clients agree to comply with prep and aftercare instructions for best possible outcomes. All treatments come with informed risks, client compliance is key.

Products

Products for sale online, special order, and in-store.  Oustanding balances on orders not paid within 5 days will be billed to card secured on file. See Store for details.  No returns nor exchanges on skincare.

Property

Access to the private, licensed  Clinic property is granted by appointment only. Attending clients to  arrive at the specific  assigned appointment time only and proceed directly to the Clinic via signs and directions.  We ask that clients do not share address to friends and family for drop-ins.  All visitors must be preauthorized time and date. Please take care to adhere to driveway and path to clinic, not to disturb neighbors or residents nor visit the private residence or wander attached property at any time. Large dogs are on  premises, enter at own risk.  Property under 24/7 security and monitoring trespassers are reported. Failure to follow directions may result in referral out and loss of any prepayments. 

Observers

Please do not bring children, pets or observers. For safety, application takes place in a controlled, sterile environment. This is a lengthy procedure, that requires the focus of the clinician, and client comfort/privacy.

Clients may bring headphones/phone for entertainment during  application. 

Food and Drink

Eat a light meal, and arrive hydrated and with a water bottle. The consumption of food and drink is prohibited in the clinic out side of water. 

Photographic Rights

Clinical photos are taken of every client procedure. Our Standard quoted pricing  reflects  discounts issued in consideration of the inherent educational and  promotional value of client before and after photos. Face Master holds the right to publish before during and after photos of all client procedures given this Pricing, these photos may be taken in-clinic or submitted by the client. Photos will be cropped to industry specs by request for increased anonymity. Not all photos or clients or treatments are shared. 

Model photos are not cropped/case studies and are subject to different contract terms.

*client may request - in advance of treatments only - that  photos remain in clinical file only. Subject to additional contract - Treatments of this type will NOT qualify for Standard quoted discounted prices. This option is offered at  our discretion* 

Records

We maintain clinical records on-site, and are kept in consideration of HIPAA regulations. Clinical photos are required either taken in clinic or by client. Please refer to Photographic Rights. 

No Guarantee

Due to the nature of biology and procedure,  pigment implantation/retention, level of therapy results, and longevity are not predictable nor guaranteed. We can estimate, but not exactly predict, healing, colors and results per client.  Refer to  Informed Consent Agreement.  Transparency in consultation and adherence to pre and post instructions  is vitally important. 

Risk

Expectation is discussed with and reasonable outcomes are communicated with all clients. Every procedures carries inherent risk. Risks include those associated with trauma to the skin, such as swelling, bleeding, bruising, prolonged healing or redness, infection and scarring and pigmentary changes. Other risks include how our own biology responds, such as  unpredictability of pigment implantation, color,  retention or longevity. Occasionally a client will change their mind or become dissatisfied on colors or placement or procedures chosen, or their body's reactivity or retention... normally due to abandonment of treatment, lack of timely establishment or poor  maintenance. 

These risks are outlined in  Informed Consent Documents, and  discussed with the clinician.  All clients are deemed to have accepted all Terms of Service, and any and all risks associated with these procedures. 

Right of Refusal

We hold the ultimate right, at any time, to deny or cancel any service at the clinician or clinic's professional discretion. If card on file is declined or expired, appointment will be cancelled.

Referred Out: Clinic may  cancel appointment or treatment plans  of clients who have not followed  instructions, do not abide by Terms of Service, or, whose expectations/goals or current health is not ideal for the chosen procedure. Non-compliant clients will be cautioned on the risks of non-compliance, and may be referred out.

Clients with adverse medical issues may require medical clearance prior to any work, we always defer to medical professionals. 

We cannot guarantee availability of future appointments, specials or maintenance sessions. Complimentary sessions may be offered or cancelled at clinic discretion with no cash/refundable/transferrable value and no future obligation.

We have a zero-tolerance policy against abuse with no refund. Spam, hate speech, hostility, vulgarity, fraud or claims of fraud etc are not tolerated and will be cancelled/blocked without notice or further obligation.. 

Health Board and FDA

Cosmetic products, ingredients, procedures and devices (those that are not intended to diagnose, treat or cure abnormal conditions or health problems) are traditionally not approved by these bodies. 

Again, they do not endorse these cosmetic applications, and may caution or warn consumers against use of: Tattooing, Microneedling, BB Glow, Pigments  to be implanted the skin, and many other cosmetic dermal procedures and applications. 

All clients receive Informed Consent documents, which states risk and accept these terms of their own free will. 

Our clinical technicians are fully trained in paramedical procedures,  certified and insured. We do not offer superficial Salon Cosmetology Services at all. No hair, makeup, nails, waxing etc nor any service that falls under cosmetology regulations. We do not, at any time, offer to diagnose, treat or cure any disease, nor do we represent ourselves as medical experts. We do not offer surgeries, nor injections.

Covid and Sickness Protocol

The clinic follows all current regulations regarding covid precautions, and may institute additional precautions for the health and safety of both clients and staff. 

In the event of  closure due to covid regulations, appointments will be rescheduled. Sick individuals must take 5 days isolation from the date we are notified. Client with covid must also  test negative prior to a new appointment. We may, subject to currently available space, allow one reschedule within 2 weeks if adequate notice is provided, as per Cancellation  Policy above. Contact

We ask all clients and potential clietns to use our 2-way text or email messaging, available during business hours only. Additional clinic and administrative closures are posted to facebook page. Messages sent  after hours are not delivered. Social media messages are not routinely monitored and do not provide customer service for appointments or related requests. Cancellations must be made the client through their accounts,  via the link  provided in confirmation emails and texts.
